Recently, an Guipeng, deputy chief physician of the Department of Cardiology and Gu Xinghua, chief physician of the Department of Cardiology, Qilu Hospital of Shandong University led a team under the guidance of Professor Chen MAO and Professor Feng Yuan of West China Hospital of Sichuan University. The first case of transcatheter aortic valve implantation (TAVR) through vena cava approach in Shandong province (the third case in China) was completed.TAVR is currently the mainstream treatment for patients with severe aortic stenosis, and femoral artery approach is the preferred approach for TAVR.However, for some patients with slender femoral artery or severe calcification, the feasibility of TAVR via alternative approaches, including subclavian artery, carotid artery or chest (including ascending aorta and apex), should be evaluated due to the small effective inner diameter.Generally, patients with poor femoral artery approach also have poor conditions in the subclavian and carotid arteries, while ascending aorta and apical approaches are often difficult to accept due to trauma and additional risks. Therefore, TRANSvena cava TAVR is the most ideal and feasible treatment for these patients. It is reported that the operation procedure of TRANSvena cava TAVR is relatively complicated, which requires high technical level and tacit cooperation of the surgical team. Currently, only 2 similar operations have been successfully carried out in China. This time, Qilu Hospital successfully implemented the first trans-vena cava TAVR in Our province (the third case in China), marking a new level of technical level of the team.Source: Dazhong Daily
